Your spot will not be reserved until payment is received in its full amount. A refund will be provided, net of any payment processing fees for any cancellations occurring before August 31, 2025. After August 31, 2025 any notice of cancellation will not result in a refund.

Elevate Officiating Academy Release of Liability

I, the undersigned, hereby acknowledge and agree that participation in the Elevate Officiating Academy NCAA 3 Umpire Training Camp involves inherent risks that may result in injury, illness, or other adverse outcomes. I understand these risks include but are not limited to physical activities, outdoor and weather-related risks, and other associated dangers.

In consideration of Elevate Officiating Academy permitting me to participate in camp activities:

1. Assumption of Risk: I assume all risks associated with participation, whether known or unknown.
2. Release and Waiver: I release, waive, and discharge Elevate Officiating Academy, its staff, directors, agents, academic institutions, and affiliates from any and all liability for any claims of injury, illness, or damages that may result from participation.
3. Medical Consent: I authorize Elevate Officiating Academy to seek emergency medical treatment for me if necessary and agree to be responsible for any medical expenses incurred.
4. Indemnification: I agree to indemnify and hold harmless Elevate Officiating Academy, its staff, directors, agents, academic institutions, and affiliates from any claims arising from my participation.
5. Photo/Media Consent: I give consent for Elevate Officiating Academy to use photographs or videos taken during the camp for promotional purposes.
